We are seeking a dedicated and compassionate Healthcare Professional to join our team in Southampton, Hampshire. In this role, you will provide high-quality care and support to our adult female client ensuring their comfort, safety, and well-being with active participation in goal-directed rehabilitation interventions as well as helping her with her executive functions.

Working Hours:
Can be flexible on the amount of hours in package depending on your needs ranging from 20 hours - 40 hours
Key Responsibilities:
- This will include supporting the client in functional daily living activities, supporting with organisation, and planning, and supporting her communication needs, both spoken and written.
- She will need help to draw up realistic plans and schedules, prompts to record and check information, and help to get things done.
- Thinking supports are also needed, including support to link information together to make sound decisions and to solve problems.
- Her rehabilitation is currently co-ordinated by a case manager with input from a multi-disciplinary team to address on-going physical, cognitive, and linguistic processing difficulties.
Requirements:
- Previous experience in a similar role, preferably in complex care.
- Excellent communication and interpersonal skills.
- Compassionate, patient, and empathetic nature.
- Ability to work independently and as part of a team.
- Flexibility to work the specified hours.
